Medical Cannabis
Utah qualified medical provider for initial and or renewal cards. The patient can register themselves on the EVS utah gov website. Once registered the patient makes an in person visit with me. This is a qualification visit. After the visit is complete. Let me know that the patient has registered and I will certify the patient in EVS. The Center for Medical Cannabis emails the patient with further instructions to activate the card.

Can print the card same day during visit as long as patient has a qualifing condition.

Patient will receive an email from Center for Medical Cannabis immediately after certification with payment instructions. 15 dollars.

Qualifiying conditions:Aids, ALS, Alzheimer's Disease, Autism, Cachexia, Cancer, Crohns' Disease, Debilitating seizures, Epilepsy, HIV, Hospice, MS, Nausea, Pain-persistent, persistent muscle spasms, PTSD, Rare condition, Terminal illness, Ulcerative Colitis, Acute Pain
